In the late 15th century, there was a small stone pillar.
It housed an image portraying the Virgin who worked a miracle on 27 March 1483. The lords of Imola Girolamo Riario and Caterina Sforza had the sanctuary built in 1491.
It has been attracting pilgrims since then.
It is a rectangular building in Renaissance style.
A small portico gives access to it.
Its 16th century belfry is 35 m high.
Arches supported by 12 pillars divide the sanctuary’s two aisles.
Above the altar stands the famous image of the Madonna.
